Please take a minute to print and fill out the Patient Registration and Health History and Dental History forms before your first appointment:

• Patient Registration and Health History Form*  PDF
• Dental History Form for patients 18 and older**  PDF

*The Patient Registration and Health History Form is the same form we will have you complete once every two years as a Health History Update in our office to update your information.

**Current patients and patients under the age of 18 will not need to complete the Dental History Form. Returning adult patients (those seen more than 3 years ago or longer) would need to complete the Dental History Form.
